1. Consent text
By checking the box before submitting the order, in accordance with Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as the "GDPR"), which entered into force on 25 May 2018,
I consent to the collection, storage and processing of my personal data provided by me to the administrator.
2. What data will be processed
First and last name, or company name, email address, telephone number, IP address, to the extent provided in connection with the order of the Operator's products and/or services, or in the context of negotiations on concluding an agreement with the Operator, as well as in connection with the concluded agreement, for the purposes specified below in paragraph 3.
3. For what purposes will personal data be processed and stored
3.1. Personal data, including:
first and last name, or company name, email address, telephone number, IP address, to the extent provided in connection with the order of the Operator's products and/or services, or in the context of negotiations on concluding an agreement with the Operator, as well as in connection with the concluded agreement, will be collected, stored and processed by the administrator for the following purposes:
for sending commercial and marketing offers and administrator notifications.
4. Storage period of personal data
The administrator will store personal data for the required time. The maximum storage period for personal data is 20 years.
5. How will personal data be processed and stored?
Personal data will be processed: automatically (machine-based) using computers and computer programs.
6. Rights of the data subject, information and access to personal data
As a data subject, I have the right to access my personal data, the right to rectification or deletion, and the right to restriction of processing. I have the right to request information about which personal data is being processed and the right to an explanation regarding the processing of personal data.
In addition, I have the right to object to processing, as well as the right to data portability (i.e. the right to receive personal data concerning me that I have provided to the administrator in a structured, commonly used and machine-readable format, for more details see Article 20 of the GDPR).
I have the right to withdraw my consent at any time, without affecting the lawfulness of the processing based on the consent given before its withdrawal, if the processing is based on Article 6(1)(a) or Article 9(2)(a) of the GDPR. This means that I do not have such a right, in particular, if the processing is necessary for the fulfillment of a legal obligation incumbent on the administrator (especially obligations related to LVTČ).
As a data subject, I have been informed that I can withdraw my consent to provide data by filling in the form on the website at mr-consult.eu or by sending an e-mail to kancelar@mr-consult.eu.
As a data subject, I have the right to request information about the processing of my personal data, and the administrator is obliged to provide me with this information without undue delay. The content of the information is determined by the provisions of Article 15 of the GDPR. The administrator has the right to request a reasonable fee for providing the information, which does not exceed the costs necessary to provide the information.
As a data subject, in case of doubts about compliance with the obligations related to the processing of personal data, I have the right to contact the administrator or the Personal Data Protection Authority.
The administrator declares that no automated decision-making takes place during the processing, including profiling referred to in Article 22(1) and (4) of the GDPR.
The administrator declares that personal data are not processed for scientific or historical research purposes or for statistical purposes.
The administrator declares that the personal data will be processed: automatically (machine-based) using computers and computer programs.
7. Final provisions
As a data subject, I declare that I am aware of my rights in accordance with Chapter III of the GDPR.
I declare that all data provided is accurate and truthful and is provided voluntarily.
The administrator declares that it will collect personal data to the extent necessary to achieve the purpose and will process them only in accordance with the purpose for which they were collected.
